Concussion Management and Return to Play Forms

The safety of our players is Lebo Hockey's #1 priority, and we follow USA Hockey's concussion management protocols.  Click the link below for details about concussion management.

In order to return to play after sustaining a concussion, a player must submit the forms below to the Lebo Hockey registrar at vp@lebohockey.org.

Players are required to submit a signed physical form prior to participating in season activities.  The form is available by clicking the "PIAA Physical Form" button below.  The form must be signed after May 31 and submitted electronically to the Mt. Lebanon High School by clicking the "Submit Physical Form" button below.
